What is Flat Feet (Adult-Acquired / PTTD)?
Adult-acquired flatfoot deformity (AAFD), also known as posterior tibial tendon dysfunction (PTTD), is a progressive condition where the posterior tibial tendon becomes inflamed or torn, leading to gradual arch collapse. This is one of the most common causes of adult flatfoot.
Causes
Several factors can contribute to the development of flat feet (adult-acquired / pttd):
- Posterior tibial tendon inflammation or tear
- Overuse or repetitive stress
- Obesity
- Diabetes
- Hypertension
- Age-related tendon degeneration
Symptoms

- Pain along the inside of the ankle
- Gradual flattening of the arch
- Swelling on the inside of the ankle

- Pain that worsens with activity
- Foot turning outward progressively
