Welcome to Our Practice
Thank you for choosing Infinity Foot & Ankle for your podiatric care. We want your first visit to be as comfortable and efficient as possible. Please review the information below before your appointment.
What to Bring
- Photo ID (driver's license or government-issued ID)
- Insurance card(s) -- front and back
- Referral or authorization (if required by your insurance)
- List of current medications and dosages
- Previous medical records or imaging related to your foot/ankle condition
- Completed new patient forms (available online)
Arrival & Check-In
Please arrive 15-20 minutes before your scheduled appointment time to allow for check-in and paperwork completion. If you have completed your new patient forms online, you may arrive 10 minutes before your appointment.
Cancellation Policy
We require at least 24 hours advance notice for appointment cancellations or rescheduling. Failure to provide adequate notice may result in a cancellation fee. We understand that emergencies happen -- please contact us as soon as possible if you need to make changes to your appointment.
